Use this form to let the team know that you require an update to your Firstname or Lastname, and to add an additional name that you would like us to use on your certificates and badges etc.

For example, if you have a maiden name that you would like on your certificates, add something like ‘(nee Richardson)’. Please make sure that the text you enter here is accurate, spelt correctly and not all lowercase and not all uppercase!

Your additional name will appear under your name on the certificates and badges. If you require a reissue of a badge or certifcate, please contact the ELC team.
